The Wastewater Management Division is pleased to make our application forms, instructions and fee information available in electronic formats. All documents listed on this page can be downloaded to your computer.

Microsoft Word forms allow you to enter information in the blanks, print and save a completed form with your information entered or print the blank form.

To move from field to field within a form, use your mouse or press Tab. Some of the form fields are checkboxes - use your mouse or the spacebar to mark and unmark them.

Some form fields limit the amount of room you have to work with. If additional space is needed to respond to a question, please attach a separate page. Number the response on the attachment to correspond with the number of the question on the application form.

Wastewater System and Potable Water Supply Permit application forms must be sent to the appropriate regional office for processing. All other applications are to be submitted to the Waterbury address. Please check the following items before mailing because the application package will be returned to you if it is incomplete:

· Have all the questions been answered?
· Has the correct person signed the form?
· Are supporting documents and plans enclosed?
· Is the correct fee enclosed? Is the check made payable to the State of Vermont?

General Permits for Discharges From Petroleum Related Remediation Activities

.

A general permit for discharges from petroleum related remediation activities became effective October 15, 2002. Another permit authorizing discharges to municipal wastewater treatment facilities is effective July 15, 2004. Applicants who wish coverage under either of these general permits must apply on a special application (a 3-part form available in hard copy only) and must satisfy a 10-day public comment requirement.

The appropriate application form and additional information may be obtained by calling 802-241-3822.

Shoreham Wastewater Treatment Facility, a recirculating sand filter, designed to treat 35,000 gallons per day. The facility was started in 2001.
